How to Get Reimbursed for Guava Using Your HSA/FSA

If you use a Health Savings Account (HSA) or Flexible Spending Account (FSA), you may be able to get reimbursed for your Guava purchase when it's recommended by a licensed healthcare provider. Many HSA/FSA providers accept what's called a Letter of Medical Necessity—a form that explains how Guava supports your health.

What’s a Letter of Medical Necessity?

A Letter of Medical Necessity is a statement from your healthcare provider explaining why a specific product is recommended as part of your medical care. It's often used to support reimbursement for tools that help manage, prevent, or alleviate a condition—particularly when they aren't automatically covered by HSA or FSA plans.

How to Use the Attached Template:

  1. Download the PDF attached below.
  2. Give it to your provider (doctor, therapist, nurse practitioner, etc.).
  3. Have them complete and sign the form to confirm that Guava supports your care.
  4. Submit the letter and your receipt to your HSA or FSA provider.
  5. If approved, you’ll be reimbursed by your HSA or FSA administrator.
