How to Use the Attached Template:

- Give it to your provider (doctor, therapist, nurse practitioner, etc.).
- Have them complete and sign the form to confirm that Guava supports your care.
- Submit the letter and your receipt to your HSA or FSA provider.
- If approved, you’ll be reimbursed by your HSA or FSA administrator.
